Payments:
Payment for services provided is expected on the day of your appointment. Our office accepts the following methods of payment: cash, check, Visa, Mastercard and Discover.
If you are eligible, you can finance your treatment using CareCredit. We have CareCredit applications available in our office or you can apply online. Also, we now work with Chase Health Advance. This will allow you to have some or all of the necessary dental treatment done, while you make low monthly payments with a reasonable interest rate. Always feel free to contact our office to find our more information on these options.
Insurance Accepted:
Dental insurance is designed to defray the costs of dental care by covering only a portion or percentage of the bill. The benefits that you will receive are based on the terms of the contract that were negotiated between you or your employer and the dental insurance company, not your dentist office. Keep in mind that the dental insurance companies are not concerned about coverage for optimal care, but provide coverage for a limited number of basic services. Unfortunately, some of the dental services that you may need or want will not be covered by your dental insurance carrier.
We do accept all PPO dental insurance plans, however we are not "in network" with all of the companies. You can always check with your individual plan to see if Advanced Dental Solutions is listed as a "in network" or "preferred provider".
To make sure that you have an optimal dental treatment for your individual condition, the treatment suggested to you will be based on your dental needs. It will not be dictated by your insurance company and their limited coverage. As a courtesy to our patients, on the day of your appointment you will only be charged the estimated co-pay and we submit your insurance claim for the entire amount on your behalf. Because many times there is such a discrepancy between the amounts insurance companies will pay for procedures, after insurance payment, you may still have a balance. It is your responsibility to pay that balance.
Our dentist office will make every effort to help you understand and make the most of your dental insurance benefits, however there is no way for our office to know everything about all the different insurance plans. It is the patient's responsibility to know their own plan benefits. Please let us know if you have any questions or concerns.
Appointment Cancellations:
If you need to cancel or reschedule your appointment at our Crystal Lake dental office, we kindly ask you to give us a 48 hour notice, so we have time to schedule another patient in need of care. Without 48 hours notice prior to your appointment, a charge of $25 charge may be applied to your account.
